National Momentum for Health & Wellness Coaching
Health & Wellness Coaching is entering a historic moment. The National Board for Health & Wellness Coaching (NBHWC) is actively engaged with the Centers for Medicare & Medicaid Services (CMS) to advance national recognition, access, and reimbursement for board‑certified coaching.


Recent NBHWC efforts include:
• Multiple meetings with CMS leadership and staff
• Submission of formal letters and executive summaries
• Collaboration with 80+ healthcare systems, employers, researchers, and clinicians
• Strengthening the case for coaching as an evidence‑based, reimbursable service

This work builds on more than a decade of coordinated research and policy efforts; and momentum is accelerating.

How Coaching Fees Can Be Covered
Use HSA/FSA Funds
Many clients can use their Health Savings Account (HSA) or Flexible Spending Account (FSA) to cover coaching fees.
 
How it works:
• Your physician completes a Letter of Medical Necessity (LMN)
• You submit the LMN and receipts to your HSA/FSA provider
• Requirements vary by plan
 
I provide a draft LMN template to make this simple and clear.


Health & Wellness Coaching is recognized by the American Medical Association with Category III CPT codes:
These codes support insurance review and reimbursement in certain settings.
While Medicare has not yet established written coverage policies for these codes, systems such as the Veterans Health Administration are already using them.
 
NBHWC continues to work directly with CMS to advance reimbursement pathways, with additional updates expected in 2026–2027.

• You may request that your physician submit coaching sessions using these CPT codes
• Some systems already reimburse
• Broader coverage is expected as CMS continues its review

FAQ: HSA/FSA & CPT Codes

Can I use my HSA or FSA for coaching? 
Yes — if your physician recommends coaching for a medical condition.
 
What do I need from my doctor? 
A Letter of Medical Necessity (LMN). I provide a draft template.
 
Can coaching be covered by insurance? 
Coaching now has AMA‑recognized CPT codes. Coverage is emerging, and some systems already reimburse.
 
What if I don’t have a medical condition? 
HSA/FSA and CPT codes apply only when coaching supports the prevention, treatment, or management of a medical condition. Personal‑growth coaching is paid out of pocket.
